    Forms and Record Keeping

    This page contains tools for members to use during the 4-H year. All information can be downloaded to your computer and saved for future use.

    The following information is helpful for members who are enrolling in 4-H:

    Note: The 4-H Participation Form is an annual form needed from all 4-H members to participate in the program and/or any state 4-H event. Adults attending state events must also complete this form and turn it into the Extension Office.

    Record Keeping

    This is a life skill. Sedgwick County members should complete the following forms each year. All these records should be kept in your 4-H record book or similar notebook.

    Note: This form has expansion capabilities. As you add information, the cells expand. Members simply add to this record each year. It is inteneded as a summary of their 4-H and other experiences.

    Note: This form is to be completed new each year. The 4-H Story page located on the back of the personal page is written annually by the member. You do not have to use a specific form. It may be hand written or typed. For more tips on writing a 4-H story ....click here

    Note: Complete one form for each project.

    Kansas Award Portfolio (KAP)

    If a member chooses, they may submit a project portfolio at the end of the 4-H year. This application will be judged and used to select project award winners at the annual 4-H Recognition Banquet in the fall.

    Sedgwick County has a simplified form for members 4-H age 7-13.

    For members who are 14 and over, use the state KAP form.

    The top winner in the senior category (14 and older) in each project will also be eligible to compete for area awards.

    Pin and Award Applications

    4-H members may achieve a lapel pin each year by meeting standard criteria. Members should do the pins in order, applying for one pin a year.

    County Awards

    These applications are due to the Extension Office by August 31 each year and awards are presented at the 4-H Recognition Banquet in November.




    Sedgwick County 4-H Ambassadors

    This group promotes 4-H to the general public, and also serves as spokespersons and liasons to local 4-H clubs. Past activities have included: narrating at the banquet and 4-H fashion show, and hosting a lock-in for younger 4-H members. The group meets about 6 times a year. Members must be at least a freshman in high school to apply. In general, applications are due October 1.
